The more dentists, physical therapists, diagnostic tech’s, doctors and specialists one sees (especially if you are traveling between one or more homes), the more scattered your medical records and your risk of medical and drug errors. Here’s a suggested checklist and start with your next visit by asking for and keeping a copy of your test results. Keep records in a safe, familiar and easy to spot place. Always carry a mini-version, about the size of an index card with you.
Health Records Checklist
- Personal ID’s – name, contact info, passport & driver’s license number
- Emergency Contacts – names, locations, contact details
- Physician Contacts – telephone numbers for physicians, specialists & dentist
- Legal & Health Insurance Information
- Group, Pvt, and Government Insurance Plans & Policy Numbers
- HIPPA Statements – who’s authorized to know and talk?
- Living Will – what you want done and not done (advance directive, power of attorney)
- Organ Donor – authorization, if any
- Personal and Family Health History
- Current Medications – Medicine names, dosages, pharmacy
- Immunizations – dates, types, reasons
- Significant Conditions/Illnesses – surgeries, plus dates
- Allergies – food, medicines, flowers, other sensitivities, etc.
- Test Results – diagnostic and lab tests, plus, dental, eye exams
- Latest Physical Exam Results
- Specialist Opinions
